Activity Consent

This document explains the participation consent requested when an adult books a child into an activity provided by the club.

Declaration shown during booking

I confirm that I am the child’s parent or legal guardian, or that I have been authorised by a person with parental responsibility for the child, to make this booking, provide the information required, and consent to the child taking part in the activity, as explained in the Activity Consent document.

What you are confirming

By actively ticking the activity consent confirmation, you confirm that:

  • you are the child’s parent or legal guardian, or have authority from a person with parental responsibility;
  • you are authorised to make the booking and provide the information needed for it;
  • you consent to the child taking part in the activity identified in the booking; and
  • the booking details, including the activity, date, time, location and attendee, are correct.

Participation and safety

Activities are delivered using reasonable safety measures, but ordinary physical play and group activities may involve risks such as trips, falls, bumps or contact with equipment or other participants. The child should follow reasonable safety instructions given by authorised staff.

This consent does not exclude or restrict any responsibility that cannot lawfully be excluded, including responsibility for negligence. Any activity-specific restrictions or requirements shown in the booking information also apply.

Care information and emergencies

Medical, allergy, medication, dietary, access and emergency information is covered by the separate Safeguarding & Care Data Agreement. In an emergency, staff may take reasonable immediate steps to protect the child and contact the emergency contact or emergency services.

Record of consent

the club keeps a booking record of the confirmation, including its date and time and this declaration version. Historical bookings are not marked as confirmed where this confirmation was not captured.

Changing or withdrawing consent

Contact the club before the activity if authority or participation consent changes. Withdrawing participation consent means the child cannot continue to be booked into or take part in that activity. Cancellation and refund rights remain governed separately by the Terms of Service.
